What to bring with you to your first Medical Massage appointment:
1. Prescription from your physician for Massage Therapy
If Auto Insurance Claim:

If Workers' Compensation Claim:
2. Acceptance of Claim
3. Employer's mailing address
4. Employer's EIN (tax ID number), found on your pay stub.
Your first appointment will consist of information gathering, such as reviewing your doctor's prescription and diagnoses, having you complete paperwork regarding your medical history and accident; I will ask you questions about your accident and injuries, and make sure you have completed and signed all the appropriate paperwork so that I can bill your insurance. I will then provide testing to complete my evaluation:
1. Balance check. I may have you stand in front of a grid and put each foot on a weight scale (that you can't see) to check for any visual and/or weight imbalances.
2. Range of motion testing. I will ask you to move your neck, shoulders, arms, back and legs to see if your injuries have caused any impairments to your range of motion.
3. Gait observations. I will observe how you carry your body when you walk.
4. Muscle testing. I may test the strength of certain muscles, and provide orthopedic tests to rule out certain contraindications and precautions to treatment. If you test positive to certain procedures, I will referred back to your physician for further testing and diagnosis prior to Massage treatments.
We will then plan your treatment options and schedule your prescribed appointments. I will also discuss with you what to expect in the normal course of casualty claims process, as well as Oregon laws regulating casualty insurance and Medical Massage.
